Work Environment\n

This role offers a modern remote work setting, allowing you the flexibility to connect with clients and colleagues while maintaining a strong focus on individualized care. While your primary work will be conducted from home, you will also engage in regular visits to clients in the Silverthorne area, ensuring a personalized touch in our service delivery. Our working hours are Monday through Friday from 8:00 AM to 4:30 PM, with occasional evenings or weekends for client needs. Flexibility is essential, and on-call availability may be required for urgent situations.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs that encompass emotional, mental, and social factors influencing their well-being.

• Collaborate with clients and their families to develop tailored care plans that reflect their personal goals and values, ensuring an individualized approach to care.

• Provide counseling and emotional support to seniors, effectively aiding them in managing life transitions such as bereavement, health changes, or relocation.

• Advocate for seniors' rights and access to necessary community resources, ensuring they receive appropriate healthcare, social services, and support.

• Work collaboratively with an interdisciplinary team, including nurses, caregivers, and medical providers, to coordinate comprehensive care plans for clients.

• Perform regular home visits to assess clients’ living conditions, monitor their well-being, and address potential issues impacting their safety and comfort.

• Facilitate effective communication among family members involved in the care of seniors, resolving conflicts and ensuring a collaborative approach to support.
\n
• Educate clients and families about available community programs, support groups, and resources that enhance social engagement and improve their quality of life.

• Maintain accurate and detailed documentation of client interactions, assessments, care plans, and progress notes in compliance with regulatory standards.

• Stay updated on best practices and regulatory changes in social work to ensure compliance with ethical and professional standards.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, qualifying you for licensure.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• Thorough understanding of social work principles, aging-related issues, and familiarity with community resources in the Silverthorne area.

Compensation and Benefits\n

We offer a competitive salary range of $60,000 to $100,000 per year, directly commensurate with your level of experience and qualifications.
