Position Summary

The Physical Therapist Assistant (PTA) works under the direction and supervision of a licensed Physical Therapist to provide therapeutic treatments and rehabilitation services that help residents achieve their highest level of functional independence.

Essential Responsibilities

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Accurately complete all required documentation in accordance with facility, state, federal, and payer requirements.
• Maintain a safe treatment environment through adherence to infection control and safety standards.
• Participate in care plan meetings, interdisciplinary team meetings, and rehabilitation conferences as needed.
• Collaborate with nursing staff and assist with training restorative nursing aides.
• Communicate resident progress, goals, and discharge planning updates with the supervising therapist and interdisciplinary team.
• Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety awareness, mobility techniques, and fall prevention.
• Recommend and coordinate appropriate durable medical equipment including walkers, canes, splints, grab bars, and other assistive devices.
• Assist with quality assurance initiatives and rehabilitation program development.
• Provide facility in-service education and training as needed.
• Maintain confidentiality of resident and facility information.
• Build positive relationships with residents, families, staff members, and healthcare partners.

Qualifications
Required Education & Experience

• Associate Degree from an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, post-acute care, rehabilitation, or long-term care preferred.
• Proficiency with electronic medical records and healthcare software such as Net Health and PointClickCare preferred.

Licensure & Certifications

• Current Physical Therapist Assistant (PTA) license in the State of California.
• License must remain active and in good standing.
• Current CPR certification if required by state regulations or facility policy.
• Ability to meet ongoing continuing education requirements.
