You'll provide education about urologic conditions, procedures, and ongoing care, helping patients understand their treatment plans and feel prepared for each step.You'll coordinate appointments and procedures, support patients managing chronic urologic conditions, and arrange follow-up care after emergency department visits. Possess the ability to develop and implement community education, outreach programs/workshops and seminars as defined for assigned specialty area required.* Possess ability to comprehensively assess patient's psychosocial status and demonstrate an understanding of the physiological disease practice as defined for assigned specialty area(s) required.* Demonstrated ability to work collaboratively with Primary Care teams and/or multidisciplinary care teams required.* Promotes excellence through setting high standards and providing high quality outcomes.* Ability to act with integrity, kindness, and exhibit empathy.* Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.* Commitment to working in a team environment and maintaining confidentiality as needed.* Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.* Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.* Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.**Job Specific Duties:*** Evaluates the needs and coordinates the care of the patient through the continuum of care by ensuring patient needs are met.* Works closely with members of the multidisciplinary care team and their recommendations. Exhibits professionalism and accuracy in interfacing with providers, clinic staff, patients, and family members involved in the plan of care.* Documents interaction per professional standards related to pre/post care and/or survivorship plan as applicable. Assesses the physical, psychological, social and spiritual needs of the patient and family.* Identifies and assesses the learning needs of the population under care and/treatment. Builds therapeutic and trusting professional relationships with patients, families, and caregivers. Utilizes motivational interviewing techniques to assist patients in meeting goals and managing chronic disease as defined for assigned specialty area(s).* Assists with the organization of the specialty interdisciplinary team meetings and facilitates the team's recommendations in a professional and timely manner.* Assists with specialty tests per standard and procedure as defined for assigned specialty area(s).* Works collaboratively with all stake holders to identify patient needs and assists in care and services, both medical and social, throughout the continuum.* Collaborates with the navigation team and leadership to optimize processes and metrics to; reduce avoidable ED visits, inpatient admissions, hospital readmissions, cost of care, gaps in care, increase patient satisfaction, clinical outcomes, and provider satisfaction as defined for assigned specialty care area(s)* Assesses education needs and seeks to update knowledge base to provide quality and current patient specific specialty service line coordination.* Facilitates the use of telehealth and/or self-monitoring technology to enhance the plan of care and patient engagement with the assigned specialty care team as needed.* Actively participates in activities that add value to organizational objectives and workflows, such as process improvement projects and/or community-related initiatives to include community health fairs, workshops, etc.The above essential functions are representative of major duties of positions in this job classification.
