Outpatient Physical Therapist

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in alignment with the patient's needs, goals, and physician directives. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that promote effective reintegration into community activities.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to establish appropriate treatment plans and goals to facilitate patient discharge within the established timeline.
• Maintain accurate and timely medical documentation, including goal writing, quality assessments, and letters of medical necessity.
• Provide educational support to patients, families, students, and colleagues.
• Participate in student mentorship programs, ensuring effective learning environments.
• Develop a personalized professional development plan and pursue ongoing education and self-improvement.
• Adhere to the State Practice Act and regulatory standards to maintain professional licensure.
• Support organizational performance improvement initiatives to elevate service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification.
• Active state licensure as a Physical Therapist is required.
• Prior experience with the agency is preferred.

Schedule

This position offers a 5x8.0 schedule, with 40 hours per week.
