Hospice Social Worker

At ViaQuest Hospice we take a holistic approach, offering services to treat the physical, emotional and mental needs of our patients and their loved ones. Responsibilities may include: Performing comprehensive patient assessments. Providing patient visits as scheduled. Providing short term crisis intervention and individual or family counseling as needed. Utilizing our electronic medical record system, Brightree, to complete documentation. Serving as a patient and family unit advocate. Requirements for this position include: Must have Masters degree in Social WorkLicense in Social Work from State of Ohio preferred. Experience in Medical Social Work is preferred. Experience within the Hospice field is preferred. Valid driver's license, car insurance and reliable transportation.
