Social Services Assistant

DAY IN THE LIFE AS A SOCIAL SERVICES ASSISTANT

As a Social Services Assistant for our elder care facility, you will be asked to assist with coordinating any resident changes such as room transfers, cable, pharmacy, etc; and notifying family members as needed. You will also coordinate staff training sessions such as Residents’ Rights, Guest Relations and Resident Dignity. Social Service Assistants also assist in coordinating Discharge Planning Service (placement options, discharge times, special needs etc.) and initiate follow up calls within 48 hours. Most importantly, you go home knowing you made a difference in the lives of others!

QUALIFICATIONS

Previous experience in a long-term care facility and MDS documentation Degree in Social Work or related field Do you have excellent interpersonal, marketing and communication skills? Are you patient and emotionally mature? Are you open to working independently and also collaborate as a team player? Do you have a proved track record in sales and business development? If so, you might just be perfect to join our skilled nursing team as a Social Services Assistant!
