Job Summary

Join a dedicated team as a Registered Nurse (RN) Case Manager specializing in Home Health, serving communities across Vermont and New Hampshire. This role involves delivering comprehensive nursing, rehabilitation, hospice, and personal care services directly to patients in their homes, fostering deep patient relationships and contributing to their overall well-being.

The RN Case Manager is responsible for screening, evaluating, re-evaluating, and treating patients, as well as coordinating and communicating treatment activities with an interdisciplinary team and the patient's support network. This position offers broad independence while being an integral part of a cohesive patient-care team, ensuring a holistic approach to patient management.

This assignment is based in the White River Junction, Vermont area, providing essential home health services to residents in the surrounding communities.

Key Responsibilities

• Develop and periodically review patient care plans in collaboration with the patient's physician, the patient, and their primary caregivers upon admission and throughout the care continuum.
• Coordinate the patient's care plan through effective interdisciplinary communication and case conferences, ensuring timely advisement of discharges to other disciplines.
• Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems, monitor progress toward desired goals, and communicate significant changes to the patient's physician and interdisciplinary team.
• Accurately document evaluations, treatment goals, and objectives in compliance with professional and organizational standards.
• Educate patients and caregivers on self-care techniques, medication management, dietary guidelines, and other physician-ordered instructions, utilizing opportunities for health counseling.

Required Qualifications

• Graduate of an accredited School of Nursing.
• Minimum of two (2) years of recent experience in a Home Health setting.
