Description The Occupational Therapist provides competent, courteous and respectful evaluation, treatment and supervision of patients for difficulties with mobility, cognition, function, strength, positioning, and activities of daily living in accordance with AOTA scope of practice and licensure laws. The OT communicates with families, physicians and other health team members, maintains professional documentation of services in the medical record, conducts in-services and training for patient caregivers and facility staff on an ongoing basis with regard to restorative and functional maintenance programs, and reports to the Therapy Program Manager and ultimately to the Director of Operations. Responsibilities include: Assess patients for rehabilitation services within 24–48 hours of physician orders. Develop and implement effective treatment plans. Treat patients according to the treatment plan of care. Supervise the Occupational Therapy Assistant in delivery of clinical services. Communicate patient progress and plans with physicians, managers and other health team members. Participate in patient plan‑of‑care meetings and interdisciplinary team meetings. Meet or exceed minimum productivity standards established by CRI. Provide in‑service training to facility staff. Document treatment notes, progress notes, discharge summaries, and complete daily billing. Instruct patients’ families or nursing staff in maintenance programs. Participate in discharge planning; recommend equipment as needed. Report equipment problems to appropriate personnel. Act as clinical supervisor for affiliated occupational therapy programs at universities and colleges. Comply with occupational therapy licensure and certification standards. Consistently portray the mission, vision, core values, and professional image of Concept Rehab. Perform special projects and other duties as assigned. Required Qualifications B.S., Master’s or Doctorate in Occupational Therapy. Licensed as an Occupational Therapist in the state(s) where services are performed.
