Responsibilities:

• Provide compassionate and skilled care services to clients in their own homes, including assistance with daily living activities, medication management, and mobility exercises
• Develop and implement personalized care plans in collaboration with our clinical team to meet each client's evolving needs
• Maintain accurate and detailed records of client care, including progress notes, medication administration records, and incident reports
• Collaborate with family members, caregivers, and healthcare professionals to ensure seamless coordination of care
• Participate in ongoing training and education to stay up-to-date on the latest caregiving techniques and best practices

Requirements:

• High school diploma or equivalent required; higher education in a healthcare-related field preferred
• Previous experience in home care or a related field; training in caregiving techniques and best practices a plus
• Possess a strong understanding of client-centered care principles and a commitment to delivering exceptional customer service
• Ability to work independently and as part of a team to ensure seamless delivery of care services
• Possess excellent communication and documentation skills, with the ability to effectively communicate with clients, family members, and healthcare professionals
