One Love Agency is hiring full-time Behavioral Health Case Managers and Psychosocial Rehabilitation Specialists for our Salt Lake City office, starting immediately.

Who we are

For more than fifteen years we have walked with families through the hardest seasons of their lives. Six offices now, across Idaho and Utah. People in crisis. Families rebuilding. People finding their way back from addiction. We serve the folks other systems have the hardest time holding onto.

We run on a Hawaiian word: hānai — to take in, to raise as your own. That is how we treat our clients and it is how we treat each other. When someone joins One Love, they join a family.

We also work like a village. The case manager, the PSR specialist, the therapist, the peer support specialist, the front desk, the biller — every one of us is standing in a circle around a client. Nobody is the star and nobody is support staff. If the front desk misses an eligibility check, the best hour you ever spent with a client never gets to count. We each carry a piece, and we carry it for each other.

And we hold to three things we call the Big C's :

Clients. They are the reason for every decision we make. Not the schedule, not the paperwork, not convenience. Them.

Compliance. A note written well is proof a person was actually served. It is how we protect a client's access to care, protect the staff who provide it, and protect the agency that employs them. We stay at 100% — not because someone is watching, but because the people we serve cannot afford for us to slip.

Community Partners. Clinics, courts, schools, shelters, housing authorities, recovery programs, DWS. They send us people they are responsible for, and that trust is on loan. We do what we said we would do, and when we can't, we tell them the truth early.

The work

As a Case Manager (Targeted Case Management) you will help adults and youth with serious mental illness, co-occurring substance use, or developmental disabilities get to the things that actually change a life — medical and behavioral health care, housing, benefits, employment, education, and natural supports. You will assess strengths and needs, build the person-centered plan with the client and the people who matter to them, connect and coordinate the services, advocate when a system will not move, and track whether life is actually getting better — adjusting the plan when it isn't.

As a PSR Specialist you will do the hands-on skills work: teaching, modeling, and practicing the daily living, social, coping, symptom-management, and community-integration skills named in the client's treatment plan — in the office, in the community, and wherever the skill actually has to work.

Both roles sit inside a multidisciplinary team with our therapists, prescribers, peer support specialists, and administrative staff, under the clinical supervision of a licensed mental health therapist. And both roles carry the documentation — timely, accurate, tied to the treatment plan, and built to hold up under a Medicaid, PMHP, or ACO review.

We are not looking for someone to fill a slot. We are looking for someone who has been in this industry, looks at a caseload and a community and says I've got this — and then owns it. Someone who knows the Salt Lake Valley's resource map because they have actually worked it. Someone whose name means something to the clinics, the courts, and the housing programs. Someone who will grow what we can do here because they see the people we are not reaching yet.

Serve the client. Own the caseload. Take care of our partners. That is the job.

What you will carry

• Comprehensive, strengths-based assessment and person-centered service planning, built with the client and their support network
• Coordination and warm hand-offs to healthcare, housing, benefits, employment, education, and social services
• Advocacy — helping people navigate systems that are hard to navigate alone
• Skills development and rehabilitative support delivered to the goals in the plan
• Ongoing monitoring of progress, with plan revisions when the plan is not working
• Crisis support and safety planning within your scope, in coordination with your supervisor and the clinical team
• Documentation and records that meet Utah Medicaid, PMHP, ACO, and HIPAA standards — accurate, timely, and complete
• Real collaboration with the rest of the village

What we are looking for

Honestly? Someone who gets it, wants it, and can carry it.

Required

• An active Utah Behavioral Health Case Management Certificate in good standing — or an active Utah clinical license that qualifies you to provide these services. We are looking for a qualified, certified Utah case manager who is ready to carry a caseload from day one.
• Licensed candidates — SSW, CSW, LCSW, ACMHC, CMHC, LMFT, or RN — are strongly encouraged to apply and may qualify for expanded scope
• Ability to complete the Utah DHHS Office of Background Processing background screening and be approved before direct client contact, per Utah requirements
• Clear of any state or federal exclusion list (OIG LEIE / SAM) — we verify at hire and monitor on an ongoing basis, as Medicaid and Medicare participation requires
• Valid Utah driver's license, reliable transportation, and current auto insurance
• Comfort with electronic health records and a real commitment to same-week documentation

What makes you stand out

• You have done this work. Case management, PSR, CBRS, or equivalent community-based behavioral health experience — you are not learning the job on our clients.
• You know this valley. You have a working map of Salt Lake County's resources — housing, benefits, treatment, recovery, food, transportation, legal — and standing with the people who run them. When you make a call, people pick up.
• You know how the system moves. Eligibility, authorizations, medical necessity, service limits, PMHP and ACO expectations. You see the compliance issue before it becomes a recoupment.
• You are solid on documentation. Your notes are on time, tied to the plan, and hold up under review — because you understand that clean charting is how a client keeps their access to care.
• Experience serving people with serious mental illness, co-occurring substance use, or developmental disabilities
• Bilingual or multilingual ability — a genuine gift in this role, though not required
• Trauma-informed, culturally humble, and steady when a day goes sideways

The practical details
