About myPlace Health myPlace Health was founded in 2021 by mission‑aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value‑based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish. Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All‑Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place. myPlace Health is building a mission‑driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast‑growing, dynamic team as we prepare to scale our mission to serve more markets. Join Us as a Healthcare Sales Specialist and Make a Difference! Are you passionate about creating meaningful connections and empowering the underserved communities of Los Angeles County? As an Healthcare Sales Specialist (internally known as an Outreach Specialist) with myPlace Health, you’ll play a vital role in helping frail older adults live independently and maintain a high quality of life. In this dynamic role, you’ll engage with community partners, build impactful relationships, and educate individuals, families, and caregivers about the transformative benefits of the PACE program. Your work will directly support our mission of enabling seniors to thrive in their homes and communities. If you’re driven to serve, inspire, and create lasting change in the lives of others, we’d love for you to join our team! What does success look like as an Outreach Specialist? Knowledge of Eligibility Processes: Experience working with state and county eligibility workers to support enrollment efforts. Tech‑Savvy: Familiarity with CRM software is a plus, though not required—we’re happy to provide training! Passion for Service: Previous experience working with frail and aging populations, showcasing your compassion and commitment to enhancing their quality of life. Self‑Starter Attitude: A highly motivated individual who thrives with minimal supervision and takes initiative to achieve goals. Educational Background: A bachelor’s degree in healthcare, Business Administration, Marketing, or a related field is preferred but not mandatory—equivalent experience is warmly welcomed. Driving Requirements: A valid California driver’s license, reliable personal transportation, a good driving record, and auto insurance are required to support travel within the community. Language Skills: Bilingual proficiency in a language commonly spoken in Los Angeles (such as Spanish, Korean, Cantonese, Mandarin, or Vietnamese) is highly valued but not required. What are other pertinent information for this role (environment, events, etc.)? Community‑Focused Role: This position is primarily field‑based, working in vibrant community settings such as government offices, resource centers, senior centers, tiny home communities, and other areas ideal for generating referrals. Participant‑Centered Tours: As part of our commitment to delivering a white‑glove experience, you’ll occasionally host tours at our center for prospective participants, brokers, hospital social workers, and other professionals to build trust and connections. Hands‑On Support: You’ll need the ability to transport and store marketing materials independently, as well as lift up to 25 pounds when necessary. Hybrid Role: This position offers a hybrid schedule, balancing time between in‑office responsibilities and field‑based outreach in the Greater Los Angeles market. Community‑Focused Presence: Up to 80% of your time will be spent in various community settings, including government offices, resource centers, senior centers, tiny home communities, and other locations ideal for generating referrals. Flexible Schedule: This role is primarily Monday–Friday during standard business hours, with occasional evening and weekend availability needed to support community events, broker mixers, and enrollment activities. The Physical Requirements of the role Frequent local travel (most days): Ability to drive throughout the service area to provider offices, hospitals, post‑acute facilities, senior housing, and community‑based organizations—including loading/unloading outreach materials from your vehicle. On‑your‑feet work is the norm: Ability to stand and walk for extended periods during community events, health fairs, presentations, and center tours (often 3–6+ hours in a day with breaks). Event setup & breakdown: Ability to lift, carry, push, or pull outreach supplies (e.g., signage, brochures, table materials), typically up to 25–30 lbs, and perform frequent bending, reaching, and crouching when setting up or breaking down booths. Driving + admin follow‑through: Ability to sit for periods while driving between locations and completing timely documentation, CRM updates, emails, and follow‑up calls between visits. Clear communication in busy environments: Ability to speak and present to individuals and small groups—sometimes in noisy or crowded settings—while maintaining a professional, welcoming presence. Comfort with outdoor conditions: Ability to work outside during community outreach events and tolerate varying weather (sun/heat, cool temperatures). Frequent use of mobile/office tools: Ability to use a phone, tablet, and computer throughout the day for navigation, scheduling, CRM documentation, and communication; repetitive hand/finger motions for typing and texting. Reasonable Accommodations: We’re committed to creating an inclusive workplace. If you need a reasonable accommodation to perform the essential functions of this role, please let us know—we’ll engage in an interactive process and work with you to identify an appropriate accommodation. $61,700 - $78,000 a year What’s In It for You?
