Develop, establish, and administer a treatment plan with specific goals set based on patient capacity and tolerance under physician direction. Set realistic, achievable goals; document and record patient condition. Educate patients and families on appropriate physical therapy methods. Develop a plan of care for each patient. Provide skilled physical therapy services/interventions per physician orders. Assess patient needs, plan, evaluate, and modify care to meet therapy goals. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Doctorate degree in Physical Therapy OR a Bachelor’s degree in Physical Therapy with NPTE certification. Graduated from an accredited school. Minimum one year’s experience. Current state license as a Registered Physical Therapist / PT or actively pursuing licensure. Current CPR certification. Other requirements as determined by client facility.
