Job Description & Requirements

Requirements:
PharmD degree (required)
Active Utah Pharmacist License (required)
Minimum 2+ years of inpatient pharmacy experience
Experience with IV preparation, infusion, and order verification

Strong communication and teamwork skills
• Preferred Skillset/Experience: ambulatory care, surgery
• Common Diagnosis/Treatment: Pneumonia, UTI, Cellulitis, AFIB, CHF, STEMI, N-STEMI, Stroke
• Personality Fit: Team oriented, high focus on safety, flexibility, excellent communication
• Required Education: Pharm D
