Geriatric Social Worker

POSITION OVERVIEW:

Under the supervision of the Administrator, the Geriatric Social Worker provides case management, counseling, advocacy, resource coordination, and support services to older adults and their families. The position requires independent judgment, confidentiality, and a commitment to enhancing the well-being of older adults

KEY ESSENTIAL FUNCTIONS:

• Provide social work and case management services to older adults and their families.
• Conduct assessments, develop service plans, and monitor participant progress.
• Provide counseling, referrals, advocacy, and crisis intervention as needed.
• Assist participants in accessing benefits, housing, healthcare, transportation, and community resources.
• Maintain accurate case records and enter documentation into required electronic systems.
• Develop and facilitate workshops and activities that support mental health and wellness.
• Provide monthly reports and quality assurance metrics as required.
• Collaborate with staff, healthcare providers, and community partners.
• May be required to assist and provide coverage at other Older Adult Centers operated by the organization on an as-needed basis.
• Perform other duties as assigned.

REQUIREMENTS:

• Master's degree in social work (MSW) from an accredited college or university.
• Current NYS Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required and must remain in good standing throughout employment.
• Minimum of two (2) years of experience working with older adults, case management, mental health services, or related human services programs preferred.
