We are seeking acompassionate, organized, and proactive Care Coordinator to join our growingremote healthcare team. In this role, you will serve as a key point of contactfor patients, helping them navigate their care journey, stay engaged intreatment plans, and achieve better health outcomes.

Location: Remote (Candidates residing in Connecticut, Delaware, Florida, Maine, Maryland,Massachusetts, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island,Vermont, Virginia, West Virginia, or Washington, DC preferred.)

Requirements:

  • High school diploma or GEDrequired; Associate degree or higher preferred.
  • Experience in healthcarecoordination, patient support, health coaching, care management, medical officeoperations, or related healthcare roles.
  • Candidates must be willingand able to work within the designated time zones required to support businessoperations and team collaboration.

  • Comfortable conductingstructured follow-up conversations focused on motivation, behavior change, andtreatment adherence—not solely appointment scheduling.
  • Strong organizational skillswith exceptional attention to detail and reliable follow-through.
  • Ability to manage multipleresponsibilities within structured and evolving workflows.
  • Excellent written, verbal,and interpersonal communication skills.
  • Ability to workindependently and effectively in a remote environment.
  • Proficiency with digitalcommunication, documentation, and workflow management tools.

Preferred:

  • Experience in telehealth,digital health, musculoskeletal (MSK) care, physical therapy, or rehabilitationsettings.
  • Experience supportinginterdisciplinary care teams.
  • Spanish languageproficiency.
  • Familiarity with patientengagement, care management, or value-based care models.
