Discharge Planner

Job TitleProvide assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility.BenefitsWe offer a full suite of benefits ranging from:Competitive hourly ratesHourly rate is negotiable depending on years of experience, education, and other factors. The general hourly pay range for this role is $15-$19.14.This role does offer a shift differential of $3/hour for hours worked at night.Medical, Dental, and Vision InsurancePaid HolidaysPaid Time OffHSA & FSACompany Paid Basic Life & AD&DDisability401(k) Retirement Plan - with company matchEmployee Assistance Program/Employee Discount ProgramEssential FunctionsDevelop continuing after care plan based on patient's needs, to promote long term health and relapse prevention.Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.Act as a liaison between referral sources, patients and counselors.Ensure all patient discharge planning documentation is clear, concise and timely.Other FunctionsPerform other functions and tasks as assigned.Education/Experience/Skill RequirementsAssociates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.One or more years' experience in a clinical setting preferred.One or more years’ experience in discharge planning preferred.Licenses/Designations/CertificationsIf applicable, current licensure as a LPN, RN within the state where the facility provides services.CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).First aid may be required based on state or facility requirements.We are committed to providing equal employment opportunities to all applicants for employment regardless of an individual's characteristics protected by applicable state, federal and local laws.
