Deliver vital care to patients in a home health setting as a Registered Nurse specializing in case management. This contract opportunity runs for 8 weeks, with the possibility of extension based on census. You will provide skilled nursing care to individuals in their homes, traveling throughout the Greensboro and High Point areas. This role requires autonomy, clinical expertise, and excellent time management as you coordinate care and maintain communication with patients, families, and the wider care team.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM
• Day shifts, five 8-hour shifts per week

Key Qualifications:

• Active RN license, unencumbered
• Minimum of 1 year of case management experience
• Prior home health experience required
• HCHB and travel nursing experience highly preferred
• Must have your own reliable car for daily travel
• Strong documentation and communication skills
• Ability to handle a variable caseload independently

Essential Responsibilities:

• Provide direct nursing care to home health patients, conducting assessments and following care plans
• Case manage a designated workload throughout Greensboro and High Point
• Collaborate with multidisciplinary teams to deliver comprehensive patient care
• Complete and maintain thorough and timely documentation in HCHB system
• Educate patients and families on disease processes, medications, and care strategies
• Travel to patient homes throughout the day as required
