• Schedule: 5 shifts per week
• Shift Duration: 8-hour days
• Hours: 40 hours per week
• Shift: Day shift, 8:00 AM - 5:00 PM
• Work Setting: On-site

Job Requirements

• Active Nevada LMSW license required at time of submission
• Master's Degree in Social Work or Master of Science in Social Administration required
• At least 1 year of social work experience required
• Experience in a healthcare or hospital-based social work setting preferred
• Strong communication, care coordination, discharge planning, and patient advocacy skills
• Ability to work effectively with interdisciplinary healthcare teams in a dynamic clinical setting

Responsibilities

• Provide social work support to patients and families within the Neurology setting
• Assess psychosocial needs and develop appropriate care plans and interventions
• Coordinate discharge planning and connect patients with community resources and support services
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to support continuity of care
• Advocate for patients and families while helping navigate complex healthcare needs
• Maintain timely, accurate documentation in accordance with facility and regulatory standards
