We are looking for a compassionate, motivated, and dependable Licensed Social Worker who is excited to grow with our company. Responsibilities

• Conduct psychosocial assessments for participants.
• Develop, implement, and update individualized care plans.
• Provide counseling, emotional support, and crisis intervention as needed.
• Coordinate with physicians, nurses, therapists, and other interdisciplinary team members.
• Assist participants and families with community resources, benefits, and referrals.
• Maintain accurate and timely documentation in accordance with New Jersey Department of Health regulations.
• Participate in interdisciplinary care plan meetings.
• Support participant admissions, discharges, and transitions of care.
• Ensure compliance with all Adult Medical Day Care regulations and standards.
• Foster strong relationships with participants, caregivers, and referral sources.

Qualifications

• Bachelor's or Master's Degree in Social Work from an accredited program.
• Current New Jersey Social Work License (LSW preferred; CSW/LCSW also welcome).
• Experience working with seniors, disabled adults, healthcare, rehabilitation, nursing homes, or adult day care is preferred.
• Excellent communication and interpersonal skills.
• Strong organizational and documentation abilities.
• Compassionate, patient, and team-oriented mindset.
