Shift Type: Day shift, 8:30 AM - 5:00 PM, plus 2 weekends per month
Hours per Week: 40
Candidate Pay Rate: $81.90 per hour

Roles & Responsibilities
- Coordinate care for patients across the acute care setting from admission through discharge
- Partner with physicians, therapists, social workers, and nursing staff to build safe, realistic care plans
- Guide patients and families through next steps, resources, and follow-up needs
- Keep documentation accurate and timely in Cerner
- Advocate for patients and remove barriers that slow down their recovery
- Jump in as a trusted resource for your unit and help keep the whole team moving forward
Qualifications
- Active California RN license
- Current BLS Certification
- LA City Fire & Safety certification
- At least 2 years of RN experience
- Prior California Dignity Health or CommonSpirit experience is required
- Solid, hands-on experience in care coordination
- BSN is preferred but not required
Education
- Charting System: Cerner
- Unit Notes: 10/19 and 11/2 orientation only
Knowledge & Skills
- Care coordination and case management know-how
- Acute care experience that you can put to work right away
- Comfort navigating the Cerner charting system
- Strong communication and patient advocacy skills
- The ability to juggle priorities with a calm, steady approach
- Teamwork and a collaborative spirit
