Position OverviewWe are in search of an advanced practice provider, specifically a nurse practitioner (NP) or physician assistant (PA), who has experience in primary care or internal medicine, ideally with a focus on geriatrics. This role offers the chance to enhance outcomes for residents in long-term care by working collaboratively with a team of healthcare providers and case managers. Our providers primarily operate within nursing homes or assisted living facilities, caring for residents enrolled in specialized Medicare Advantage plans. Your responsibilities will include delivering personalized, coordinated healthcare that enhances quality of life and minimizes unnecessary hospital admissions.
As an advanced practice provider, you will utilize your independent judgment to manage patients with both acute and chronic conditions, consulting with a supervising physician when necessary. You will collaborate closely with our facility partners and health plan leadership to implement our national care model while adhering to all federal and state regulations.
Key Responsibilities- Evaluate patients’ medical and mental health requirements, creating a care plan that proactively addresses their conditions and overcomes barriers to care.
- Ensure compliance with local, state, and federal regulations concerning the clinical services you provide.
- Prescribe medications and order laboratory tests, diagnostic procedures, and consultations.
- Monitor patients’ adherence to treatment plans and adjust those plans as necessary.
- Collaborate with RN case managers as part of an integrated care team.
- Licenses: FNP, AGNP, or PA license is required.
- Education: Degree from an accredited APRN or PA program.
- Experience: Minimum of 3 years in clinical nursing or rehabilitation within geriatric populations.
- Technical Skills: Proficiency in Electronic Health Records and familiarity with Microsoft applications, including Word, Outlook, and Excel.
You may be expected to provide training and guidance to facility staff as needed.
