RN Case Manager: Care Coordination & Discharge Planner

IU Health in Indianapolis, IN seeks an experienced RN – Case Manager to plan and coordinate patient care from pre-admission through post-discharge. You will ensure appropriate levels of care, discharge planning, and collaboration with a multidisciplinary team. You will work with third-party payers to verify coverage, assist with denials and appeals, and educate patients and families on post-discharge care. Strong communication and regulatory knowledge are essential. #J-18808-Ljbffr
