Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 Weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 Year License: Physical Therapist Professional License in Massachusetts Certifications: None specified Must-Have: 1 Year Home Care Experience Description: Provide physical therapy services to patients in their place of residence. Patients may have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Collaborate with patients, families, physicians, and other allied health professionals to identify patient needs. Coordinate services to meet identified patient needs. Utilize thorough and timely electronic documentation. Engage in interdisciplinary communications in accordance with agency policies and standards of practice. Weekend rotations may be required, occurring once every 6 weeks. Ability to travel to assigned cases is necessary. 1 Year of Certified Home Care experience is required. 1 Year of Case Management experience is also required. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement.
