Qualifications To excel in this role, you'll need: Licensure: A valid LGSW or LISW license in the state of Minnesota (Master's-Level Social Worker). Experience: At least one year of clinical Social Work experience, with proficiency in Electronic Medical Records (EMR) preferred. Responsibilities As a valued member of our team, you'll: Conduct home visits to assess patients' psychosocial status and needs. Provide a range of Social Work services in the home, including short-term individual counseling, community resource planning, and crisis intervention.
