Medical Coordinator

Overview LTFC is seeking a compassionate and detail-oriented Lead Medical Coordinator to join our dedicated team. This role plays a vital part in supporting the health and well-being of children and families through direct care coordination and community-based services. Location and Schedule Location: 843 Whalley Ave, New Haven, CT Schedule: 8:30 am - 5:00 pm, this position requires flexibility and has varied hours Organization The Children's Community program of CT Key Responsibilities Coordinate medical care and services for children in our programs Conduct home visits to assess and support client needs Escort children to and from medical appointments Collaborate with healthcare providers, case managers, and families Maintain accurate documentation and ensure compliance with care standards Required Education and Experience Licensed Registered Nurse (RN) or bachelor’s degree in a health-related field (e.g., Physician Assistant, Nurse Practitioner) Minimum of 1 year of experience in public health or healthcare settings Demonstrated experience in care coordination, health administration, or direct clinical care Supervisory experience and licensure to provide clinical services in the state of employment Bilingual proficiency in English and Spanish Experience conducting home visits and supporting children during medical transitions Seniority level Entry level Employment type Full-time Job function Health Care Provider Industries Individual and Family Services #J-18808-Ljbffr
