Responsibilities

• Provide individualized physical therapy treatment plans for patients.

• Conduct thorough assessments to evaluate patient needs and progress.

• Set and achieve personal performance goals aligned with company targets.

• Document patient progress and treatment outcomes accurately and timely.

• Stay updated on the latest physical therapy techniques and practices.

• Engage in continuous learning opportunities to enhance professional skills.

• Collaborate with healthcare professionals to optimize patient outcomes.

• Educate patients and families on treatment plans and home exercises.

• Utilize data and metrics to track patient progress and treatment effectiveness.

• Participate in team meetings to discuss patient care strategies and improvements.

• Maintain a clean and safe working environment for patients and staff.

Experience/Qualifications

• Valid state licensure as a Physical Therapist (PT).

• Commitment to continuous learning and professional development.

• Proven experience in developing individualized treatment plans.

• Strong assessment skills to evaluate patient needs effectively.

• Ability to work in a results-driven environment with high visibility.

• Ability to document patient progress accurately and timely.

• Experience collaborating with healthcare professionals for optimal outcomes.

• Strong communication skills for educating patients and families.

• Familiarity with using data to track treatment effectiveness.

• Willingness to maintain a clean and safe working environment.
