Description

  • Assess patients in either inpatient or outpatient environments and create tailored treatment plans
  • Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
  • Collaborate closely with nurses, doctors, case managers, and rehabilitation specialists to maximize patient outcomes
  • Educate patients and their families regarding therapeutic goals, effective discharge planning, and at-home exercise regimens
  • Maintain comprehensive documentation of evaluations, treatments, and progress in line with regulatory standards
  • Participate actively in care team discussions and interdisciplinary meetings to fortify coordinated care

Education

  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program

Certifications

  • Current state licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

  • Prior experience is highly desirable

  • Excellent communication and teamwork abilities
