DescriptionSummary of Duties: This position will assist in coordinating care for Early Head Start children and their families. In compliance with Federal Performance Standards, duties will include but are not limited to: performing evidence based screenings, monitoring records, educating staff and families, conducting home visits, fostering relationships with health care professionals, and supporting both pre and postnatal care. Essential Duties and Responsibilities: Other duties may be assigned. • Monitor status of all required health components of the Early Head Start program to ensure all Federal Performance Standards are being met; • Foster community partnerships though communicate with other health professionals and interested organizations in the community to obtain their service, support, and involvement with Early Head Start; • Support Family Service Assistants in obtaining documentation for well child visit and components; • Educate families on the importance of regular well child visits and assist with removing barriers to obtaining; • Utilize strong interpersonal skills that are required to work with families, agency staff, and community or government entities; • Work with Family Educators, as requested, to provide information on health related issues; • Educate expectant parents on the importance of regular prenatal health care; • Provide staff training on health-related topics as requested; • Provide family education on health-related topics and preventative practices as requested; • Join Family Educators on home visits, as needed; • Complete postpartum visits with new mothers within the two week timeframe established by Federal Performance Standards; • Assist in coordinating and facilitating mandated meetings of the Health Services Advisory Committees (HSAC); • Complete hemoglobin testing as well as lead testing for any enrolled child unable to obtain the screen from local Health Department or physician, at 12 and 24 month. Enter screen results into the Early Head Start database, and report lead levels to the State, as required; • Perform evidence-based hearing and vision screens, as needed, for newly enrolled children within 45-day timeline and for families that express concerns; • Maintain client confidentiality at all times; • Assist with recruitment of eligible children; • Frequently travel throughout multi-county service area; • Maintain professionalism when representing the agency at all times; • Report any suspected cases of abuse, neglect, or exploitation to the appropriate agency as well as the Early Head Start Director. RequirementsEducation and Experience: Valid Licensed Practical Nurse (LPN) certification or licensure is required. Associate's degree or Bachelor's Degree in Nursing in addition to LPN certification preferred. At least one year of early childhood experience is preferred. Working knowledge of computers including Windows, Microsoft Office, email and ability to learn assessment data collection software required. Certificates, Licenses, Registrations: Must have valid Michigan LPN license, valid Michigan driver's license and reliable transportation. Language Skills: Ability to read, analyze and interpret (reports, regulations, periodicals). Write (reports, business correspondence and manuals). Present information and respond to questions. Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
