Physical Therapist (PT) - Home Health | Local or Travel Opportunity

We are seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team, delivering high-quality, patient-centered care directly in patients' homes.

This role is ideal for therapists who value independence, flexibility, and meaningful one-on-one patient care. You will help individuals regain mobility, strength, and confidence following illness, injury, or surgery-making a real difference in their daily lives.

Job Details

Start Date : ASAP

Schedule : Monday-Friday, 8:00 AM - 5:00 PM (40 hours/week)

Coverage Area : West Plains territory, including Ozark, Douglas, and Howell counties

Weekly Pay : $2,846 Gross Weekly

On-Call : May be required based on staffing and branch needs (subject to change)

Productivity Requirement : 30 units/week

Required Qualifications

• Active State Physical Therapist (PT) license
• Experience with Homecare Homebase (HCHB)
• Experience with OASIS documentation
• Prior Home Health experience
• Current CPR certification
• Valid driver's license
• Reliable transportation with active insurance
• 1-2 years experience as a PT in acute care or rehabilitation setting

Preferred Qualifications

• Previous home health or home care experience

Education

• Bachelor of Science in Physical Therapy from an APTA-approved program (required)
