Responsibilities
- Conduct initial evaluations and develop individualized plans of care for residents in a SNF setting.
- Design and implement therapeutic programs to improve mobility, strength, balance, gait, and activities of daily living.
- Provide evidence-based interventions including therapeutic exercises, gait training, ROM, manual therapy, and modality application as appropriate.
- Re-assess progress regularly, document outcomes, and communicate updates to the interdisciplinary team and care partners.
- Monitor safety, ensure proper infection control, and adjust treatment plans to meet patient goals and discharge planning requirements.
- Educate residents and caregivers on home programs and strategies to maintain gains after therapy ends.

Requirements
- Active Physical Therapist license (Connecticut) or eligibility to practice in the state
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting
- Strong clinical documentation and communication skills
- Commitment to collaborating with an interdisciplinary care team
- Ability to meet assignment-specific credentialing and scheduling requirements
