You will collaborate closely with physicians, families, and the interdisciplinary team to deliver personalized care.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansProvide therapy focused on improving ADLs, strength, coordination, and functional independenceMonitor patient progress and adjust treatment plans as neededMaintain accurate and timely clinical documentationCommunicate with physicians, residents, families, and care team membersParticipate in care planning and discharge planningConduct staff education and in-service trainingQualificationsDegree in Occupational Therapy (Bachelor’s required; Master’s or Doctorate preferred)Active, unencumbered OT license in South Carolina (required)Must meet all continuing education requirements and remain in good standing with the State BoardExperience in a skilled nursing or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong clinical, communication, and organizational skillsWhy Join Fleetwood Post Acute?At Fleetwood, you’ll enjoy the flexibility of PRN work while being part of a stable, experienced team that values your skills and supports your growth.
