Lead Navigator

Lead Navigator Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services. A Lead Navigator is a recovery‑experienced individual (at least one year in recovery) who has a minimum of 90 days experience as a Navigator or Peer Support Specialist. They work primarily with homeless individuals, providing outreach, engagement, case management, and wrap‑around supportive services to help the member end homelessness. Responsibilities The Lead Navigator serves as the bridge for the patient, coordinating and monitoring RBHA and non‑RBHA community‑based services. Key duties include: Conducting outreach and engaging individuals with the highest acuity and greatest vulnerability. Coordinating care with community providers and stakeholders, including specialty positions such as Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads. Facilitating delivery of services by supporting the individual and ensuring services are provided as intended. Mentoring Navigator II staff, onboarding them, and helping them understand program requirements. Managing processes such as coordinated entry, housing referral, leasing up, wrap‑around services, and discharge. Skills and Requirements High school diploma or GED is required. Associate’s degree (or higher) in a field related to behavioral health is preferred but not required. 1–3 years of full‑time health care‑related work experience that provided exposure to both behavioral and medical crisis situations (or equivalent combination of education and experience). Current driver’s license. Clear 39‑month motor vehicle record. Certified Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI clinical policies and procedures.
