Registered Nurse - Home Health Care

Location: Portland, Maine

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift from 8:00 AM to 5:00 PM, with every other weekend rotation or permanent schedules available based on agency needs.

Job Description: Join our dedicated healthcare team as a Registered Nurse in Home Health Care, responsible for managing patient caseloads and conducting in-home visits. You will be expected to provide quality care to adult patients with a variety of medical and surgical needs, including oncology and respiratory conditions.

Responsibilities

• Manage a caseload of 20-25 patients with an expectation of 5-6 visits per day.
• Perform comprehensive assessments and implement individualized care plans.
• Administer medications and treatments as prescribed and monitor patient progress.
• Utilize EMR systems (Epic) for documentation and patient management.
• Collaborate with interdisciplinary teams to enhance patient care and discharge planning.

Required Experience / Certifications / Licensure

• Must be a licensed Registered Nurse (RN) with at least 2 years of experience, preferably in home health care.
• Basic Life Support (BLS) certification required.
• Experience with Epic and OASIS documentation is essential.
• Preferred skills include IV therapy, tube feeds, wound care, and management of various medical devices.
