Case Worker

Busy NYC Hospital seeks an organized, detail-oriented case worker to manage social services case load.
Hospital Case Worker- Interviews, makes initial assessments & referrals.
- Manages cases of social service recipients.
- Makes home visits to adults to assess their needs.
- Investigates & verifies information obtained.
- Provides health related and social services to clients.
- A satisfactory Bachelor's Degree.
- A minimum of 1 year of recent related experience with nursing home placements.
- Outstanding communication skills and ability to meet deadlines.
