Hospice Social Worker PRN

Hospice Social Worker Location: Live Oak / Hollywood Park Bloom Healthcare is a Colorado-based health system specialized in population health management, home-based primary care, and hospice. We are seeking a Social Worker to join our growing Hospice Team and work 7-10 visits per month. Bloom Social Workers are cross‑trained to provide care in several areas including home visits, admission and imminent visits, as well as palliative care. The hospice social worker provides psychosocial support to patients and families by offering resources, referrals, education, supportive counseling, discharge planning, and assistance with final arrangements. The position requires driving city‑wide. Care delivery primarily takes place in the patient’s home or community. Weekend availability is required. Responsibilities Develop and maintain therapeutic relationships. Provide comprehensive psychosocial assessments. Develop and maintain comprehensive plan of care. Provide supportive counseling. Facilitate smooth transition from care environments. Mobilize community resources. Provide effective patient/family/caregiver teaching. Support the death/dying process. Collaborate in providing patient care. Create timely and accurate documentation. Assure patient safety. Utilize resources effectively and efficiently. Adhere to Bloom standards and continuously improve processes/services. Maintain and enhance professional skills. Adhere to high standards of personal and professional conduct. 50% of caseload home-based patients. Minimum Qualifications Master's degree in social work from an accredited school and a minimum of 1-3 years of palliative care or hospice experience. Demonstrated ability to assess emotional and social needs of patients and their families. Ability to administer psychological/emotional assessments and define treatment planning from results. Excellent communication and interpersonal skills. Ability to effectively collaborate with interdisciplinary team in providing patient care. Solution-driven, creative and resourceful problem-solving skills. Ability to function independently and effectively anticipate needs and problems. Skilled in organizing and prioritizing work. Ability to manage time well and perform assigned duties with attention to detail, accuracy and follow-through. Professional image, positive attitude, enthusiasm and self-motivation. Comfortable maintaining professional boundaries. Proficient computer skills. Broad understanding of community resources including financial benefits. Awareness and sensitivity to cultural diversity. Knowledge of self-care, personal boundaries; abilities, limits and inner resources. Current CO Driver’s license and automobile insurance. Preferred Qualifications Previous hospice experience.
