In-Home Nurse Practitioner or Physician Assistant Per Diem - Fallon

Position Overview Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You will visit patients in the comfort of their own homes, conduct medication and medical history reviews, perform physical evaluations, and, when ordered by the patient’s health plan, administer additional diagnostic tests such as diabetic eye exams and spirometry. You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests, or alter patients’ current treatment regimens. Work as a clinician colleague with Signify Health, fulfilling the requirements of your role. Be flexible to travel locally and within licensed states as needed. Conduct virtual visits when required. Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exams, bone density screening, and others. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Compensation & Benefits Hourly base rate of $41.85 to $90.13, depending on experience, education, geography, and other factors.
