We are seeking a knowledgeable, organized, and detail-oriented MDS Coordinator (RN/LPN) to support resident assessments, reimbursement accuracy, regulatory compliance, and quality outcomes.

Position Overview

The MDS Coordinator is responsible for coordinating and supporting the accurate, timely completion and submission of the Minimum Data Set (MDS) assessments in accordance with applicable federal and state requirements.

This position works closely with Nursing, Therapy, Social Services, Dietary, Activities, and facility leadership to ensure resident assessments accurately reflect each resident's clinical condition, functional abilities, services provided, and individualized care needs.

The MDS Coordinator also supports Patient-Driven Payment Model (PDPM) reimbursement, Medicare documentation, care planning, quality measures, and ongoing survey readiness .

The ideal candidate is a licensed nurse with strong clinical assessment skills, excellent organizational abilities, and an understanding of skilled nursing documentation and reimbursement processes.

Key Responsibilities

MDS Assessments & Coordination

• Coordinate the completion of comprehensive and noncomprehensive MDS assessments in accordance with applicable requirements.

• Maintain accurate assessment schedules and ensure completion within required timeframes.

• Review resident clinical information to support accurate MDS coding.

• Coordinate assessment information with Nursing, Therapy, Social Services, Dietary, Activities, and other interdisciplinary team members.

• Ensure MDS assessments accurately reflect residents' clinical conditions, functional abilities, and care needs.

• Monitor assessment completion, accuracy, and submission requirements.

• Identify documentation discrepancies and coordinate necessary follow-up.

• Maintain accurate assessment records and supporting documentation.

Medicare, PDPM & Reimbursement

• Support accurate Medicare Part A assessments and PDPM classification.

• Review clinical documentation to ensure appropriate representation of resident conditions and services.

• Collaborate with Nursing, Therapy, and facility leadership regarding skilled services and supporting documentation.

• Monitor assessment reference dates and required Medicare assessment schedules.

• Identify opportunities to improve documentation accuracy and reimbursement integrity.

• Assist with reviewing diagnoses, clinical conditions, and services relevant to PDPM classification.

• Communicate reimbursement-related documentation concerns to appropriate leadership.

• Promote accurate reimbursement practices consistent with applicable requirements.

Resident Care Planning & Interdisciplinary Coordination

• Participate in the development and review of individualized resident care plans.

• Ensure MDS assessment findings are appropriately incorporated into resident care planning.

• Identify care plan or documentation concerns requiring additional follow-up.

Clinical Documentation & Accuracy

• Review clinical records for accuracy, completeness, and consistency with MDS coding.

• Maintain appropriate confidentiality of resident health information.

Quality Measures & Resident Outcomes

• Monitor MDS-related quality measures and identify potential areas for improvement.

• Support development and implementation of appropriate quality-improvement interventions.

• Promote accurate assessment practices that support resident care and quality reporting.

Regulatory Compliance & Survey Readiness

• Maintain knowledge of applicable MDS requirements, the Resident Assessment Instrument (RAI) Manual, and relevant CMS guidance.

• Ensure assessments are completed and submitted within required regulatory timeframes.

• Assist with maintaining ongoing assessment and documentation compliance.

• Promote a culture of accuracy, accountability, and continuous improvement.

Education & Clinical Team Support

• Serve as a resource to nursing employees regarding MDS documentation and assessment requirements.

• Provide education and guidance related to documentation accuracy and resident assessments.

• Promote effective communication between clinical departments.

• Maintain professional knowledge through ongoing education and training.

Resident Rights & Professionalism

• Treat residents with dignity, compassion, patience, and respect.

• Promote resident independence, choice, and person-centered care.

• Protect resident privacy and confidentiality.

• Maintain accurate and ethical clinical documentation.

Qualifications

• Active Wyoming Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing, or other applicable authorization to practice nursing in Wyoming

• Graduate of an approved nursing program

• Previous MDS coordination or skilled nursing assessment experience strongly preferred

• Knowledge of the MDS 3.0 assessment process and RAI Manual preferred

• Understanding of Medicare Part A, PDPM, and skilled nursing reimbursement preferred

RN-specific assessment responsibilities must be completed by an appropriately qualified RN in accordance with applicable requirements.
