Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive initial assessments on patient admission
• Coordinate multidisciplinary care at both intra-hospital and inter-hospital levels
• Conduct transitional planning activities, including patient education and aftercare arrangements
• Collaborate with healthcare teams to share relevant patient information
• Contribute to goals of reducing length of stay and preventing readmissions
• Participate in and document performance improvement activities
• Review commercial payer cases, determine dispositions, and perform self-audits
• Evaluate the needs and safe discharge of geriatric patients
• Engage in all other related duties as assigned

Requirements of the RN - Case Management - Travel

• Registered Nurse (RN) NYS license
• PRI Certification to be obtained within 3 months
• 3 years of clinical experience in ED, critical care, or med/surg preferred
• Previous case management experience preferred
• BLS (AHA) certification
• Experience with Milliman/InterQual criteria
• Proficient computer skills
