Hospitalist Program Coordinator

Coordinates the daily operations of the Hospitalist Program to ensure efficient patient care, provider communication, and continuity of services. Responsibilities include coordinating physician schedules; serving as the liaison between hospitalists, referring primary care providers, specialists, hospital staff, patients, and families; facilitating communication regarding patient admissions through the Emergency Department; participating in and coordinating discharge planning activities with Case Management to support timely and appropriate transitions of care; scheduling post discharge follow up appointments; coordinating follow up of pending diagnostic tests and ensuring appropriate communication after discharge; ensuring discharge summaries are forwarded to the appropriate providers; collecting physician billing documentation; screening calls; assisting with medical records and billing processes; coordinating physician access to patients and families; supporting physician performance on quality metrics and regulatory measures through data tracking, communication, and follow up; and promoting effective interdisciplinary collaboration. Responsible and accountable for understanding and supporting the organizations vision, mission, values, and standards while contributing to high quality, patient centered care. Missouri RN License required.
