About the Role The Inpatient RN Case Manager at Sinai Hospital works with the clinical team and medical provider to coordinate and implement safe discharge plans for patients. The main goal is to improve patient well‐being and outcomes while ensuring healthcare services are used efficiently and timely. Sign‑On Bonus: This position is eligible for a $10,000 sign‑on bonus. Key Responsibilities Assessment & Planning: Conduct initial and ongoing assessments to determine patient needs, develop focused discharge plans, especially for high‑risk patients. Intervention & Collaboration: Work closely with the clinical team and medical providers to put the discharge plan into action. Continuous Improvement: Stay current with healthcare trends, regulations, and payer requirements related to patient care, discharge planning, and benefits. Requirements Basic professional knowledge, equivalent to a Bachelor’s degree; working knowledge of theory and practice within a specialized field. BSN preferred; ADN required. 3–5 years of related experience; prior experience with inpatient case management or discharge planning is required. Registered Nurse license – current Maryland license or eligibility to obtain one. Case Management certification preferred within 3–5 years of hire.
