Responsibilities

• Administer prescribed medications and treatments per physician orders and facility policy.
• Monitor patient condition, document vitals, intake/output, and report changes to the supervising nurse.
• Assist with activities of daily living, mobility, and meal support.
• Perform routine nursing procedures and wound care within scope of practice; monitor for safety and comfort.
• Maintain accurate documentation in electronic health records; collaborate with interdisciplinary team to coordinate care.
• Ensure infection control practices and patient safety at all times.

Requirements

• Active Licensed Practical Nurse (LPN) license.
• Willingness to travel and work in an assigned location.
• Availability for evening and/or overnight shifts.
• Strong patient care, observation, and documentation skills.
• Effective communication and teamwork abilities.
• Experience in or comfortable with long-term care settings.
