Description

  • Engage collaboratively with nurses, physicians, case managers, and fellow rehabilitation experts to maximize patient outcomes.
  • Instruct patients and their families on therapeutic goals, discharge plans, and suggested home exercises.
  • Keep detailed documentation of evaluations, treatments, and patient progress according to facility and regulatory standards.
  • Take part in multidisciplinary rounds and care team conferences to ensure a seamless care continuum.

Education

  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.

Certifications

  • State licensure as a Physical Therapist
  • CPR certification preferred
  • Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory.

Skills

  • Prior experience within the field is highly desired.
