Director of Case Management Case Management

Schedule: Variable Days - 40 Hrs 08:00-16:00 (Weekdays Only; may have some on-call rotations or late nights/long days)

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• 3+ years of recent acute hospital case management experience
• 2+ years of leadership experience
• Bachelor's degree required
• Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) required

Roles & Responsibilities:

• Direct and evaluate departmental operations, including staffing, onboarding, and staff competencies to achieve performance and quality objectives.
• Ensure compliance with care coordination and discharge planning processes, ensuring timeliness, quality, and proper documentation.
• Assess and improve departmental performance by evaluating operational processes, monitoring performance through data analysis, and implementing sustainable performance improvement activities.
• Establish working relationships with key stakeholders to include CMO, CNO, and ancillary service leaders.
• Engage interdisciplinary teams and external stakeholders to ensure continuity of care and optimal clinical resource utilization.
• Manage multiple priorities and adapt quickly to change in a leadership role.
