Job Description

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our senior living community. This individual will be responsible for providing high-quality nursing care, administering medications, coordinating resident care, maintaining accurate documentation, and ensuring residents' physical, emotional, and social well-being. The ideal candidate is passionate about serving the elderly population and thrives in a collaborative team environment.

Day-to-Day Responsibilities

•Administer medications according to physician orders and maintain accurate medication records.

•Complete resident assessments and assist with the development of individualized service plans.

•Monitor changes in residents’ health conditions and ensure concerns are evaluated, documented, and addressed.

•Perform nursing procedures within the appropriate scope of practice and in accordance with state regulations.

•Respond to medical emergencies and provide first aid when needed.

•Review resident records to ensure documentation accurately reflects concerns, interventions, and outcomes.

•Communicate with residents, families, physicians, nursing staff, and other healthcare providers.

•Support infection-control practices and maintain a safe, healthy environment.

•Assist with supervising wellness department employees who provide daily resident care.

•Provide orientation and ongoing training to team members as requested.

•Prepare written and verbal reports within required timeframes.

Skills and Requirements

•Active LPN license in good standing with the applicable state licensing authority.

•Ability to administer medications and perform nursing procedures within the appropriate scope of practice.

•Strong clinical documentation and organizational skills.

•Ability to recognize and respond to changes in a resident’s condition.

•Ability to work scheduled hours and remain available by phone for questions involving medical concerns. •1-3+ years of nursing experience in assisted living, senior care, skilled nursing, or long-term care.

•Experience developing resident care plans.

•Supervisory or training experience.

•Knowledge of state regulations related to senior living communities.

•Experience participating in resident and family care conferences.

•Experience with electronic medical records (EMR/EHR) systems
