Key Responsibilities\n- \n
- Engage directly with assigned clients in both virtual settings and community outreach, providing targeted support to those experiencing mental health challenges. \n
- Conduct thorough assessments and ongoing evaluations to understand clients’ needs and tailor interventions accordingly. \n
- Facilitate individual, group, and family therapy sessions, ensuring treatment plans align with clients’ presenting problems and goals. \n
- Collaborate with interdisciplinary teams to design integrated care plans and enhance the continuum of care for clients. \n
- Implement evidence-based practices in brief, focused prevention and recovery services to foster resilience and recovery. \n
- Monitor admission, continued stay, and discharge criteria, ensuring smooth transitions and continuity of care. \n
- Participate in on-call coverage as needed, providing crisis intervention and support to clients outside of regular hours. \n
- Document client interactions and maintain accurate records to ensure compliance with professional standards and organizational guidelines. \n
This position primarily serves clients dealing with severe emotional disorders and major mental illnesses in a hospice setting, emphasizing the importance of compassionate care and support. You will work with a varied population, including individuals and families facing end-of-life challenges, making sensitivity and strong interpersonal skills essential for effective communication and support.
Qualifications\n- \n
- A Bachelor's Degree in Social Work or a related field is required; an advanced degree is preferred. \n
- A valid state license as a Clinical Social Worker (LCSW) is necessary for practice. \n
- A minimum of 2 years of direct experience working with individuals experiencing mental health issues is essential. \n

Compensation and Benefits\nThe annual salary for this position ranges from $70,000 to $85,000, commensurate with experience and qualifications.
