Location: Oak Village Healthcare - Oaktown, IN
Schedule: PRN

Pay Rate: $65/hr (5-10 hrs C-PRN Weekly)*

Revital Rehab Services is seeking a dedicated and licensed Physical Therapist (PT) to provide PRN therapeutic care at our partner facility, Oak Village Healthcare in Oaktown, IN. This is a rewarding opportunity to help residents improve strength, mobility, function, and overall quality of life in a supportive, collaborative, and patient-focused environment.
Responsibilities:

• Conduct initial evaluations and create individualized physical therapy treatment plans
• Provide evidence-based, goal-directed therapeutic interventions
• Monitor patient progress, update goals, and document outcomes accurately
• Collaborate closely with the interdisciplinary care team
• Ensure all services meet Revital's high standards for quality, documentation, and ethical care

Qualifications:

• Licensed Physical Therapist (PT) in the state of Indiana
• Current BLS certification
• Strong clinical judgment and communication skills
• Commitment to compassionate, patient-centered care
