SENIOR COMMUNITY HEALTH NURSE

Senior Community Health Nurse - 64057455This position will work in all clinical service centers, family planning, STD, TB, immunizations, and HIV clinic. This includes gathering health histories, obtaining vital signs, giving injections and other medications, interviewing clients, utilizing nursing protocols when applicable, conduct blood draws, and documenting as appropriate in the electronic health record. Maintains HIV Pre/Post-Test Counseling Certification This position serves as the lead Registered Nurse in the Ryan White program and is responsible for the provision of nursing and medical case management services for all clients enrolled in the Ryan White program. Under the direction of the Executive Community Health Nursing Director, and in conjunction with the Ryan White Program Manager, this position works to ensure all clients enrolled in the Ryan White program are seen twice yearly and receive clinical services in accordance with the HIV/AIDS clinical care guidelines. This position also is responsible for ensuring nurse case management activities, including lab orders, medication changes, etc. are fully documented in the appropriate system. This position also shares responsibility with the HIV Program manager to assure that clinic schedules for the contracted providers are appropriately managed to maximize and make the best use of the contracted provider's time while at DOH-St. Johns. Assists in conducting county – wide investigations, surveillance, and follow-up of persons diagnosed with a reportable communicable disease. These duties may include, but are not limited to, home visits, doctor's office, and hospital visits. Responsible for ensuring follow-up of all reports. Ensures the integrity/confidentiality of all data sets in the program; complies with the information security protocols and Procedures (DOHP 50-10-16); security and privacy of client information is maintained; disclosure of confidential information is released only after proper authorization has been obtained; and information that is exempt from public record disclosure is maintained in a confidential manner. Ability to establish and maintain effective working relationships with othersQualifications: Licensure as Registered Nurse 464 F.S, Valid Florida Driver's License Other job-related requirements for this position: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. This position has been approved in accordance with Section 110.209, Florida Statutes, Chapter 60L-32 Florida Administrative Code and Collective Bargaining Agreements with the Florida Nurses Association (FNA) and the American Federation of State, County, and Municipal Employees (AFSCME), Florida Council 79. After being evaluated and recommended by the Director of Nursing to perform after hours and weekend on-call duties on a rotating schedule, the incumbent will carry a telephone and respond to emergencies and inquiries. The on-call nurse will make authorizations and referrals when medically necessary and appropriate. The on-call nurse will be compensated $1.00 per hour on-call on any day other than Saturday, Sunday, or a holiday. Saturday, Sunday and holidays', the on-call nurse will be compensated equal to the amount of one-fourth of the statewide hourly minimum for your classification Working hours: (A) Daily from 8:00 to 5:00 (B) Total hours in workweek 40 (C) Explain any variation in work (split shift, rotation, etc.) This position requires daily in-office or field work.
