RN – Case Management
Seven Healthcare are seeking an experienced Home Health RN Case Manager for a 13-week travel contract in Baltimore, MD. This role is within the home health specialty, covering Baltimore City, Baltimore County, and possibly AA and Howard County. The position requires working with multiple site visits per week using Point of Care/HomeCare HomeBase, and OASIS is essential.

Key Responsibilities of the Home Health RN Case Manager
- Perform comprehensive assessments of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.
- Continuously reassess cost-efficient and appropriate levels of care.
- Ensure compliance with treatment plans through non-medical support.
- Coordinate care transitions and engage specialty resources as needed.
- Maintain detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Act as a patient advocate by protecting privacy and confidentiality.
Requirements of the Home Health RN Case Manager
- Associate's or Bachelor's Degree in Nursing or related field.
- Current, unrestricted Registered Nurse license in the state of Maryland.
- Wound care experience required.
- Home health experience a must.
- Point of Care/HomeCare HomeBase and OASIS required.
- Valid Driver's License and Car Insurance required.
- CPR/BLS certification required.
- Case Management certification preferred.
