About the Position

Job Title: Case Manager Profession: LMSW Specialty: Social Services Duration: 12 months Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum 2 years of recent Case Management and discharge planning experience in a hospital setting License: California Social Work license required Certifications: BLS certification required Must-Have: - Experience in comprehensive assessment and care planning - Skills in care coordination and plan implementation - Ability to advocate for patient needs and preferences - Background in resource utilization and discharge planning Description: Coordinates the management of care and ensures optimum utilization of resources. Facilitates service delivery and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment. Engages in care planning, plan implementation, and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction. Promotes continuity of care and cost-effectiveness through integration of case management functions. Ensures overall evaluation of individual patient needs while upholding patient preferences. Serves as a patient advocate addressing the holistic needs of patients.
