Hospice Case Manager

Position Overview

The Hospice Case Manager is responsible for delivering exceptional care to patients in a home health setting. This role requires specialized experience in hospice care to effectively manage patient caseloads and coordinate comprehensive care plans tailored to individual needs.

Key Responsibilities

• Provide hospice nursing care in patients' homes, ensuring comfort and support for patients and their families.
• Develop and implement personalized care plans based on thorough assessments and ongoing evaluations.
• Collaborate with interdisciplinary teams to facilitate seamless communication and coordination of care.
• Educate patients and their families on disease progression, care options, and available resources.
• Document all patient interactions and care provided in compliance with regulatory standards.
• Ensure adherence to hospice policies and procedures to maintain quality of care.

Qualifications

• Current and valid Registered Nurse (RN) license.
• Previous experience in hospice care within a home health environment is required.
• Strong communication and interpersonal skills to engage effectively with patients and families.
• Ability to work independently and manage multiple priorities in a fast-paced setting.
• Familiarity with electronic health records (EHR) and documentation standards.

Work Schedule

This position typically involves day shifts, working five 8-hour days from 08:00 AM to 04:30 PM, with no call requirements.

Compensation

Mileage will be reimbursed at the current IRS rate for travel between patient locations.
